The core requirements across the spectrum of independent reviews (SCR, SAR, NHS SI, DHR) are the same therefore it makes sense that across the spectrum of serious case reviews performed that the process takes the best of all the approaches available to achieve:

Acknowledgment of where case management met (or exceeded) the standards required, and/or fell below these and why
Clearly articulated local learning opportunities
A dynamic local and national learning environment which optimises frontline professional and employee participation in the process In this session we will look at an approach that may assist in sharpening scope and terms of reference associated with the SAR process, and provide opportunity to think more carefully about the right approach to the SAR on a case by case basis.

Key features of interactive approaches that focus on optimal contact with staff involved in the case management

What constitutes a reasonable action plan?
Feedback on locally published and contemporary SAR report In this session attendees will feedback their thoughts on a locally published and contemporary SAR report. Chairs will be asked to reads this in advance of the workshop and come ready with their thoughts about:
Overall readability
Clarity about what: Was reasonable – good about agency case management and inter agency working Could have been improved
Fell seriously below the standards of the time and contributed to what happened in terms of lost opportunity for prevention based on what was known, what reasonable ought to have been known, and what was done ‘at the time’
The recommendations made and their likelihood of achieving measurable and sustainable change
