Progress for Providers: Two day facilitated programme. Checking your progress in delivering personalised services Purpose: To assess provider organisation’s readiness for personalisation, ascertain what needs to change in order to deliver care and support to people holding individual budgets, and to action plan next steps for implementation. This simple self assessment tool has been developed by providers and commissioners to help providers think about their progress in responding to the personalisation and Self Directed Support agenda. Aimed at: • Provider organisations supporting anyone who uses social care. • Senior managers and staff across the organisation, or in specific departmental or geographical areas Learning Points: The self assessment tool is divided into eight sections and each section represents a key area of change and development for providers as they deliver increasingly personalised services. The areas identified were developed by working with providers who have already made significant progress, and by asking them what they had needed to change. Key objectives: 1 Leadership and strategy 2 Creating a person-centred culture 3 Community focus 4 Support planning and review 5 Finance 6 Human resources 7 Marketing 8 Reviewing and improving our service
