This is a form to collect the extra information F4YP require in addition to the Holiday Activities website.

We strongly recommend all new YP sign up to an after school taster session. This can be booked by contacting the team.

EVENTS INFO

Young people will take part in a range of activities which will include arts & crafts and multi-sports - other activities may be included

Wear: sports clothes including trainers (appropriate for weather) sports will be outdoors when possible. - NO CROP TOPS.

BRING:

HAF - Lunch provided - BRING snacks & drink - NO NUTS / SWEETS OR FIZZY DRINKS ON SITE

If your YP would prefer to bring their own lunch please indicate this within the form (Lunch preference).

It can be helpful to send your YP with: Suncream & hat, a pair of named wellies, change of clothes, layers in case of poor weather. These items should be kept in a bag. They can leave this at school for the duration of the course if desired.

HAF

The government have introduced the HAF fund, which allows participants who receive free school meal benefits to attend holiday activities and receive food help.

If you are eligible for HAF, please book a HAF place through BBC website using an E-Voucher code which can be obtained from your YPs school. HAF places are free and include lunch each day. Please be aware HAF spaces are limited and we are operating a first come first served basis for both F4YP tickets and HAF tickets.

If you are entitled to HAF but do not wish to take up this offer, you can book a bursary place through our booking system which has a £15.00 cost.

BURSARY

At all our courses we have a bursary criteria which can be found here . For our summer provision our 5 day courses are £15 per young person, this cost goes towards activity provision over the summer.

F4YP can not issue a refund for any bursary tickets where a HAF ticket has later been purchased.

PAID

If you do not qualify for a bursary or HAF place we have some places that are paid tickets.

YOUNG PEOPLE SHOULD LIVE IN OR ATTEND SCHOOL IN BEDFORDSHIRE.

- If applying for a bursary place we we will require evidence to show you meet the criteria.
- Purchasing a ticket does not guarantee a place on the course. We reserve the right to refuse places to applicants.
- We accept purchase of tickets as parental consent for your YP's to receive emergency medical aid if required during his/her attendance at any of our programmes.
- All staff are independently DBS checked.
- During the course your YP may receive additional support from the pastoral team. If your child has an EHCP or additional support at school it is helpful for us to know this before hand so that we can provide consistent support. We may also contact the school if we have additional concerns. If your child has 1:1 support at school please inform us of this, we may request a visit with their school so that we can assess your child's needs.
