25/26 School/Agency Referral Form

T his form is for professionals from schools/agencies to complete for any YP they feel would benefit from accessing F4YP services.

T his form must be completed with parent/guardian consent . It gives us the information we would need for YP to start accessing our services.

I t will give you an option to suggest what clubs/services the YP would like to attend. Our team will then contact you or their parent/guardian to discuss further.

T he form will ask you to select a ticket...please choose the "School/Agency Referral - Bursary" ticket available as this will allow you to complete the form. If there are multiple YP from the same family, select the relevant number of tickets to YP and complete the form for each of them.

P lease complete the email/address etc as the YP/parent/guardian info and include your details in the referrer info question.

Please give as much information on the sign up form as possible, to help us best support your child.

• • If applying for a bursary place we will require evidence to show you meet the criteria.
  • Purchasing a ticket does not guarantee a place on the course. We reserve the right to refuse places to applicants.
  • We accept purchase of tickets as parental consent for your YP's to receive emergency medical aid if required during his/her attendance at any of our programmes.
  • We do not share information with third parties. Only statistics created from data will be used for our funding purposes. All staff are independently DBS checked.
  • During the course your YP will receive additional support from the pastoral team. If your child has an EHCP or additional support at school it is helpful for us to know this beforehand so that we can provide consistent support. We may also contact the school if we have additional concerns. If your child has 1:1 support at school please inform us of this, we may request a visit with their school so that we can assess your child's needs.
