Description

The specific purpose of this AWG is to explore what could and should be done, locally and nationally, to support positive outcomes for young people with SEND as they transition into their adult lives with a focus on health and social care support.

A self-evaluation audit tool has been written to support LAs in identifying their own practice in this area and this will be introduced to the members and tested out as part of this programme.

The AWG will address the following broad agenda/questions:


  • What do we mean by 'transition' in this context?
  • What are the positive examples we know about (nationally and locally) and what are the gaps and challenges?
  • What resources exist that we know of to promote positive transition and sharing of the PfA/transition audit tool?
  • What could be done (given what is available and the prevailing policy agenda and priorities) that is positive and possible?


Members will be asked to ‘test out’ ideas and resources to share learning at the follow-up session early next year. There may be further actions after the first meeting to engage others in this discussion e.g. further call for evidence, webinar, etc. The group will reconvene (provisionally in January), with additional participants if appropriate, to share learning so far. From this, a short report will be produced that will map out ideas for both local and national action to strengthen and develop transition.
