Advance Care Planning Training

We are running small group face-to-face training sessions on Advance Care Planning suitable for health and social care staff.

The training includes:

Understanding the principles and process of Advance Care Planning (ACP)
Identify the benefits of ACP to patients, family, professionals and carers
Developing effective communication skills
Awareness of the barriers to ACP
Understanding the implications of the Mental Capacity Act
Awareness of local resources to support individuals in the ACP planning process
Understanding of own role involvement and that of the multidisciplinary team, family and carers

This training session counts as 3 hours towards your continuous professional development and a certificate will be issued on completion.
