Hosted by trained volunteer Annette and supported by a Cardiomyopathy UK Specialist Nurse. This session is for anyone over 18 years of age with advanced heart failure/on the transplant list/transplanted.
Many of those diagnosed with cardiomyopathy live very full and active lives. Realistically though, some will experience heart failure, and a smaller number go on to develop advanced heart failure.
Wherever you are on such a journey, a holistic and personalised assessment is required and up- titrating your medications and a possible device implant can help. However, a smaller number will need to go on to be considered for a heart transplant and when the time is right - not too soon, nor too late - for the Transplant Waiting List.
This online group is for anyone with advanced heart failure who might in time be approaching the need for transplant, for people already on the list, and for those who have made the journey through to transplant. It means we have an opportunity to have a thoughtful chat and share our own journey, doing so over coffee. We want to provide a welcoming and supportive environment and to hear where you are in the process, for you to share your concerns, and so we can learn together from each other’s experiences. Our Specialist Nurse is available on group to advise as required, as we support and encourage each other along the way
Due to the complexities of the heart transplant assessment process plus considerable variations in assessment & care pathways outside of the NHS and to enable us to provide appropriate support to the attendees, we are unable to offer this group to people who do not access their healthcare within the UK.
The session will take place using Zoom - a link and password will be sent to you prior to the meeting date.
