Description

Improving outcomes for Atrial Fibrillation and Heart Failure patients

Sandwell and West Birmingham CCG are proud to announce Atrial Fibrillation and Heart Failure workshops in collaborative with West Midlands Academic Health Science Network (WMAHCN) and Sandwell and West Birmingham Hospital Trust.

The workshops will support practices to achieve the PCCF Standard 6: Cardiovascular disease.

The aim of these workshops is to highlight the role that primary care professionals can play in reducing the complications associated with a long-term condition such as atrial fibrillation (AF). Undiagnosed AF can lead to potentially devastating, debilitating and often fatal strokes, which are preventable in two-thirds of patients if they are appropriately anticoagulated. Poorly managed AF, in terms of anticoagulation, symptom control, and co-morbidities can lead to poor outcomes in the longer term.

Heart Failure (HF) is a long-term condition that often gets worse over time. It cannot be cured, but with treatment and lifestyle changes, many people can have a good quality of life. The workshop will include a brief introduction to HF diagnoses and management.

The CCG has developed clinical templates for AF and HF best practice management these will be highlighted at the workshop to support primary care.

This workshop will highlight how all sectors of the primary care workforce (GPs, nurses and pharmacists) can contribute to the detection and management of AF and HF.

The training will be from 1-3pm with lunch available from 12.30pm.

Please register for the workshops.

**3 CPD points can be attributed to your GP PDP Portfolio for attendance at this Workshop**
