The Coalition for Personalised Care (C4PC) is delighted to be working with the Anticipatory Care team at NHS England and NHS Improvement (NHSE/I) to host a user led workshop to explore what anticipatory care (AC) will mean for people, their families and carers and the communities where they live. The workshop will be an opportunity to hear from key health and social care leaders working at national, regional and local levels. The workshop discussions will help shape how the AC operating model will work in practice, how it will look and how it will be received within communities.
It will help you to understand more about anticipatory care - a proactive and coordinated approach to care and support, helping people living with complex health and care needs stay as healthy and independent as possible at home, for as long as possible. Anticipatory care is delivered and coordinated through multi-disciplinary working across health, social care, voluntary, housing and community partners, so this workshop is relevant to a wide range of participants from across all sectors.
