Anticipatory Care Planning - Everyone's Business

We’ve come a long way in moving forward with Anticipatory Care Planning. ACP is central to providing health and care services that are fit for our future and focussed on person-centred care, supported by integrated working.

This event marks a significant milestone with the launch of the National ACP documents and the ACP app and will also showcase tests of change and exemplars of Anticipatory Care Planning work being taken forward throughout Scotland.
