Join Dr Chris Jones, Sarah Fatchett and the West Wakefield Health & Wellbeing vanguard team as we present the journey so far on the road to becoming a Multispecialty Community Provider.

Learn more about the new sustainable models of care we have developed for use within primary care and community settings including:

  • Extended Primary Care
  • Care Navigation
  • Integrated Care
  • Digital delivery including Schools App Challenge, the HealthPod app and Directory

Discover the challenges we’ve faced and the results we are already seeing from this dynamic approach, centred on creating a more diverse primary care team locally.  

With opportunities to ask questions and gain real insight into our work, we hope you will be able to join us for what promises to be a stimulating afternoon.

This event will cover all of the new models of care listed above and if you have a key interest in Integrated Care, there shall be additional time spent on this topic at this session.

If you have a key interest in Extended Primary Care, you may wish to attend our event on 26 September 2016 or if it's Care Navigation that you are keen to find out about, you may wish to attend our event on 22 November 2016.

What if I need to cancel or change my booking?

If you need to change your booking once confirmed, please contact us. For any cancellations made up to two weeks prior to the date of the event, a full refund of the booking fee (where paid in advance) will be made.

If you provide less than two weeks notice of cancellation prior to the date of the event, you will be charged the full booking fee.
