NHS Hull Clinical Commissioning Group (CCG) and Hull City Council, in partnership with Hull’s Older People’s Partnership, will be holding the 2nd annual Better Care in Hull conference on Monday 26 September 2016 at the KCOM Stadium to open Older People’s Week 2016.
This free conference aims to bring together professionals from health, social care and the voluntary, community and social enterprise sector, plus a number of service users.
Following the success of the first Better Care in Hull conference, this year the partnership will look to showcase work under our refreshed Better Care in Hull schemes:
- Prevention – ensuring people live as independently as long as possible
- Intervention – ensuring people receive the most appropriate care where and when they need it
- Rehabilitation – to support people to return to daily living after a period in hospital
- Rapid Community Response – providing alternatives to hospital attendance and admissions closer to home
The conference will also provide the opportunity for attendees to get involved in collaborative workshops, giving represented organisations the opportunity to help shape the Better Care in Hull programme and make the required connections needed to work together in a more integrated way.
