Third of three Action Learning Sets BNSSG

PINCER

Medication errors in general practice are an important and expensive preventable cause of patient safety incidents associated with morbidity, hospitalisations and deaths. Given the prevalence of medication errors, and the severity of harm associated with these, there is an urgent need to implement interventions known to correct these errors. The PINCER trial was developed to study whether a pharmacist-led IT-based intervention could reduce medication error rates within the primary care setting. Published in the Lancet (Avery et al, Lancet 2012; 379:1310-1319), the PINCER Intervention was shown to be a clinically effective and cost-effective method for reducing a range of clinically important and commonly made medication errors in primary care. In conjunction with the West of England Academic Health Science Network (WEAHSN) BNSSG CCG will be hosting a series of action learning sets (ALS) that take you through the PINCER process to support the development of skills and processes in primary care in BNSSG to ensure common causes of prescribing errors are identified and resolved and processes are implemented to prevent future errors.

Each ALS will build on the previous sessions and the work completed in the practice in between, and therefore it is essential that you attend all three sessions.
