Following our productive series of discussions with NHS leaders and patient safety specialists, we are excited to invite you to our final PSIRF Roundtable of 2025. This session will be dedicated to synthesising the year's learnings and building actionable strategies to turn the PSIRF vision into reality.
Thank you to everyone who has contributed their time and insights throughout our series. Your input has revealed the barriers to effective PSIRF implementation, and more importantly, the solutions to overcome them.
What We've Learned: 4 Key Takeaways from 2025
Our roundtable discussions have highlighted four core barriers to effective PSIRF implementation and the emerging solutions to overcome them:
1. The Capacity Crisis: Time, Money, and Skill Sustainability
- Focus: Addressing the critical lack of protected time for learning by leveraging practical responses like After-Action Reviews (AARs) and focusing on embedding PSIRF skills into normal workflows to counter high turnover and limited funding.
2. Culture & Clarity: Moving Beyond Blame and Confusion
- Focus: Shifting the narrative from "incident" and "investigation" to learning responses by embedding systems thinking and developing a language of learning and openness. We will also discuss the need for clearer national guidance to resolve confusion between PSIRF and coronial processes.
3. The Executive & External Influence Challenge
- Focus: Discussing strategies for securing consistent Board-level buy-in by positioning safety as value, not cost. We will share best practices on transparent and compassionate family engagement , recognizing that family satisfaction is a powerful tool for board reassurance and external credibility.
4. Scaling Expertise: Training, Accreditation, and Collaboration
- Focus: Overcoming widespread training gaps through train-the-trainer and mentorship models. This session will explore how peer learning and collaboration can overcome resource constraints and build internal capability with consistent quality and potential accreditation.
