This event will be of interest to everyone who is committed to making person-centred care a reality – whether you work in the NHS, social care or the voluntary sector, or have experience of living with a long-term condition.
We want to support a movement of people with long-term conditions, carers and practitioners across the health and social care landscape who are determined to see big change in approaches to achieving person-centred care. At this stage in our development we want to engage with our members, anyone who shares our commitment to making person-centred care a reality and others interested in co-creating this movement. This gathering will bring together people to have creative conversations and go away with practical ideas and resources on how to deliver a new deal for people with long-term conditions.
You will have an opportunity to participate in two in-depth, practical workshops; take part in a ‘marketplace’ of person-centred care initiatives; have structured conversations to develop your plans and contribute to our next steps. You might want to bring a problem you are struggling with and sign up for a short coaching session (limited availability). You will receive a resource bag of materials from the workshops and ‘market place’.
Workshops will include:
• Building the House of Care: lessons from practice
• Personalised care and support planning: an introduction to new and forthcoming practical materials and how you can use them
• Integration and the Individual: person-centred care in the Integrated Personal Commissioning Programme
• Realising the Value: building and sharing the evidence for person centred care and its implementation
• Building Community Capacity: practical approaches for a shift to asset based approaches
• Changing a local system to deliver health and well-being for people with long-term conditions
• The contribution of health coaching to person-centred care
If you are attending as an individual patient, carer or service user, travel expenses to the event can be covered. Details will be in the joining instructions. A more detailed programme of timings and confirmation of the central Manchester venue will be released shortly.
