Care Home Support Team (BHFT)
We are a free service provided by Berkshire Health (NHS) Foundation Trust for care homes in the West of Berkshire (Newbury, Reading & Wokingham locality)
Following the Government’s initiative ‘The Dementia Challenge’, the Care Home Support Team has been set up to work alongside care homes.
The team’s uniqueness is that it is made up of experienced Mental and Physical Health nurses. Working together they support care homes and enhance their understanding of links between the two health areas which enriches the quality of care delivered and helps improve the quality of life for people with dementia. There is also a Physio, Occupational and Speech & Language Therapist working within the team.
We compliment and extend existing services provided that benefits the resident, their relatives and homes. As a team we share with the home experience, tools, training, role modelling and resources.
