Care Navigation Training Session by Age UK Islington in cooperation with CEPN
In the training we share navigation skills and knowledge of the excellent range of services and activities offered by Health, Social Care and the Voluntary Sector in Islington. This care coordination training session will help professionals in regular contact with patients and service users (with one or more long-term health conditions) to support their patients/service users to self-help and to achieve individual health and well-being improvement through the delivery of personal goals. It is about solutions, how we use linkages and coordinate. Sources and a range of learning tools will be provided.

Age UK Islington provides Care Navigation services in Islington to people with long-term health conditions (physical and mental) aged 16 years and over. Working with statutory services and voluntary and community organisations we aim to help people meet their goals, achieve a better quality of life, prevent decline and reduce individual isolation.
