Navigating Doesn't Have To Be A Nightmare
Take the stress out of navigating the plethora of referral organisations in Islington by attending a 'Care Navigation Training Course'. This FREE, half day training is tailored specifically for health, social care and voluntary sector professionals in Islington. Created by client facing staff at Age UK Islington, who know the exact same challenges you face. The session is part of the Islington CEPN training programme. Care Navigation Training Session by Age UK Islington in cooperation with CEPN
In the training we share navigation skills and knowledge of the excellent range of services and activities offered by Health, Social Care and the Voluntary Sector in Islington. This care coordination training session will help professionals in regular contact with patients and service users (with one or more long-term health conditions) to support their patients/service users to self-help and to achieve individual health and well-being improvement through the delivery of personal goals. It is about solutions, how we use linkages and coordinate. Sources and a range of learning tools will be provided.

Target Audience
Health, social care and voluntary sector professionals in Islington who want to increase their ability to navigate across health and social care provision in Islington.
