GDPR CONSENT STATEMENT

By your completion of this event booking or, if you have not booked, by your arrival and entry into the Church for either Mass or Private Prayer, you agree that St Augustine's Catholic Church Solihull, through its employees or volunteer staff, can record and use your personal data for the purpose of booking attendance at Mass or Private Prayer, managing the event of Mass or Private Prayer and complying with all applicable Covid 19 regulations and related guidance on the reopening of places of worship for Catholic Mass or Private Prayer issued by the UK Government, the Archdiocese of Birmingham and any other applicable issuing body. One of the purposes of holding this data would be to assist the NHS Test and Trace service by keeping an accurate temporary record of visitors to the Church for at least 21 days and to assist NHS Test and Trace with requests for that data if needed for contact tracing and the investigation of local outbreaks, if any is required.

The following guidance notice has been added to this booking system on 24 July 2020 following recommended advice received from the Archdiocese of Birmingham:

COLLECTING CONTACT DETAILS OF THOSE WHO COME TO CHURCH FOR NHS TRACK AND TRACE

The legal basis on which the Parish will hold this information is your consent. If you do not wish to provide your phone number for these purposes, you will still be able to attend Mass or Private Prayer. For the safety of all who attend Mass or Private Prayer, the Government has advised that a contact phone number be kept for everyone who comes to Mass or Private Prayer, for a period of 21 days. That information will be destroyed after 21 days. It will be made available to NHS Track and Trace if they contact the Parish to request that information. It will only be made available to any third party if that is required by law.

If you come to Mass or Private Prayer in a group or as a family only one contact number will be needed.

The following guidance notice issued during September 2020 has been added to this booking system following recommended advice received from the Archdiocese of Birmingham:

DIOCESAN GUIDANCE

If a person who has come to Mass/Church Service later tells a parish any of the following:

a. they were in contact with a person who has Covid symptoms (whether or not they have had a Covid test)

b. they were in contact with someone who has now tested positive for Covid

c. they have themselves tested positive for Covid*

we advise the parish to take the action set out below:-

a. contact everyone who attended the same Mass/Service and tell them what has happened, without mentioning names, and advise each person to

i. avoid contact with people at high increased risk of severe illness from coronavirus, such as people with pre-existing medical conditions

ii. take extra care in practising social distancing and good hygiene

iii. watch out for symptoms and self-isolate if they also show signs of coronavirus

b. clean the church in accordance with the government guidelines at https://www.gov.uk/government/publications/covid-19-decontamination-in-non-healthcare-settings/covid-19-decontamination-in-non-healthcare-settings

*Where a person who has come to Mass/Service later tests positive for Covid, then in addition to the above steps the parish should also:-

a. let those who attended the same Mass know that they should expect to be contacted by the NHS test and trace service and

b. provide that the NHS test and trace service with the contact details of all those who attended that Mass/Service
