Course Outline;

Course Aim:

Develop understanding of evidence base, principles of assessment and clinical actions

Course Content:

-Pain management

-Enhanced understanding of reason for failed discharges and older people

-Chronic disease management in acute settings

-Understanding complexity of caring for older people

-Dementia and cognitive assessment

-Ageing and frailty assessment

-Altered mobility and mobility assessment

-Principles of falls assessment.

-Rapid social assessment to explore the concept of discharge risks for discharge planning or transfer of care

-Understanding the Older Persons NSF

-Delivering person centred care

THIS TRAINING IS FOR THOSE WHO QUALIFY FOR CPD WHITHIN CHESHIRE & MERSEYSIDE PRIMARY CARE TEAMS ONLY.

You will be required to give details of your NMC number and Practice/PCN name.

The following Primary Care staff are eligible for CPD funding: All registered Nurses , Midwives , Art Therapists , Drama therapists , Music therapists , Chiropodists/podiatrists , Dietitians, Occupational therapists , Operating Department Practitioners , Orthoptists , Osteopaths , Paramedics , Physiotherapists , Prosthetists and Orthotists, Radiographers , Speech and language therapists.
