A one-day interactive workshop using a range of Root Cause Analysis techniques and tools in a safe environment. Led by two facilitators who have carried out investigations in a range of settings, including mental health and social care, and trained hundreds of healthcare staff in this approach.
We keep our groups to no more than 12 attendees to provide ‘hands on’ practice in investigation techniques supported by 2 experienced facilitators.
When you have completed the course you will be able to:
Recognise the human factors of error which can lead to an incident and the need to be candid with those affected
Take a structured approach to investigation
Facilitate the investigation of an incident, with the support of experienced investigators
Prepare a draft incident investigation report
Use a quality improvement approach to develop an effective action plan
Who should attend?
Clinical staff responsible for leading or facilitating incident or complaint investigations
Clinical or managerial staff who review completed investigation reports
Staff and teams who would like a refresher course to update their skills
