Discharge to Pharmacy - System Learning Event

Waltham Forest is working in partnership with Barts Health to deliver this training.

Time: 19:00pm - 21:30pm

Date: Thursday 6th June 2019

Venue: Best Western Hotel, 30 Oak Hill, Woodford, IG8 9NY - Statesmen 1

Target Audience: Pharmacists, GPs.

19:45- 20:00: Introduction to the event - Introduction to the session, outlining the objectives and how the LPP are supporting the Discharge to Pharmacy project

20:00-20:10: Introduction of DtP and the benefits - DtP overview and benefits of this service to the patients.

20:10-21:00: Demonstration of the system

  • Run through of how the system works
  • How it is utilised in practice.

21:00-21:10: CCG perspective - Commissioning of the service and impact on GPs

21:10-21:20: Skills Matrix / Digital Training Hub - Introduction and specification of the Skills Matrix / Digital Training Hub for Pharmacists

21:20-21:30: Q&A - Floor is open to questions from the delegates

Session Aims, Objective & Outcome:

CCG Medicines Team is working collaboratively across Community, Primary Care & Secondary Care/Acute to launch Innovative software called ‘PharmOutcomes’. This is an electronic prescribing system that will support care discharges to the community faster. It will also support improved communications between professionals and patients saving time whilst delivering the management of care closer to home.
