Please complete the form with your CHILD's name (or children's names in the Forename field if booking for siblings) and YOUR email address. If accepted, please deliver the £10 club fee to the office.

Please note: Clubs may be cancelled at short notice in the event of schedule changes or staff sickness. We will notify you as soon as possible in this event.
