EQ collaborative learning event: ‘Enhancing the Quality of Heart Failure Care’

The event will have presentations from the AHSN Heart Failure Clinical Leads Dr Hugh McIntyre (Consultant Physician) and Dr Richard Blakey (GPSI Cardiology) of the EQ measures data for both acute and community services across Kent, Surrey and Sussex (KSS). Shared learning from colleagues in KSS and other regions, will present on the impact on outcomes for heart failure patients when delivering an integrated care approach. Acute HF MDT Teams and CCG Leads – EQ merged with the National Heart Failure Audit in April 2015, to deliver robust monthly dashboard reports to Acute HF Services across KSS. Phase 1 - we have reported monthly on 6 key process measures and 4 fields for information only. Now in Phase 2 - we will present the new addition of outcome reporting. The dashboard will show data that links the EQ process measures with the impact on outcomes for the acute trusts. In the afternoon there will be facilitated group work to give delegates QI tools that will enable them to examine their own local data, enable transparent discussion around areas of variation and create action plans to bring back to their organisations.
Community HF Teams - ICHOM training afternoon at EQ event - Kent, Surrey & Sussex AHSN are working with volunteer Community Heart Failure Services across KSS, to begin the first phase of an International Consortium Health Outcomes Measurement (ICHOM) project, to measure Patient Reported Outcome Measures Set (PROMS) for heart failure patients in the community. This exciting opportunity will begin at the end of October and run for 9 months in the first phase. The participating teams will see the data collected for PROMS added to their existing robust EQ dashboard reports and delivered monthly.
There will be a wide variety of table exhibition stands at the event to give delegates an opportunity to explore information, innovation and technologies that are available.
