Evening Event - Integration & Partnership - A view from Age UK

We are delighted to host an evening on integrated person centred care for older people, looking at Age UK's partnership with the NHS.
Over the past few years Age UK has developed a new strategy to support better care for older people. They have tackled some of the 'wicked' issues to enable a combined medical and non-medical approach to improve the wellbeing of those living with complex co-morbidities, and sought to understand the impact of this practice - in particular the improved wellbeing of older people, the experience of receiving and delivering this model of care and the impact upon the whole system in terms of cost and activity.
If you work in healthcare integration or with older patients, this is the event for you. Join Pam Crevan and Jo Holmes from Age UK as they share the trails and tribulations of delivering a system wide approach.
Jo-Anna Holmes - Head of Integrated Care, Age UK
Passionate about person centred care, Jo heads the Integrated Care team at Age UK. Jo is a qualified Nurse, and also had a career in Housing in operational and business development roles. Before joining Age UK Jo worked as an Associate Director at the Hurley Group (GP and Integrated Care Centres). She has worked within the NHS as a commissioner for primary care and mental health alongside wider roles leading service redesign and system wide transformation.
Pam Creavan - Services and Partnership Affairs Director, Age UK
Pam has extensive experience gained from working across the private, voluntary and public sectors. She worked for many years in local government before joining the NHS as a Non-Executive Director. This led to her taking up a substantive post in the NHS leading on Partnership, Communication and Engagement. Following this as Borough Director she has responsibility for joint commissionin, including mental health services, continuing health care and services for children and young people as well as leading public health and health improvement.
The evening will also include the opportunity for networking over drinks.
