EVERY STEP COUNTS:SELFMANAGEMENT FOR PEOPLE WITH ACUTE_LONG TERM CONDITIONS

In this talk Prof Jones will present an overview of how self-management support can be used successfully as an integral part of healthcare for people living with a range of acute and long term conditions. Fiona will draw on her work developing an approach called ‘Bridges Self-Management’. Bridges co-produces self-management tools for people with different conditions, training for healthcare professionals as well as working across organisations to implement and evaluate the impact of working in a person centred way. Research strongly supports the need to find ways to individualise and tailor self-management approaches. Prof Jones will share some of the innovations developed using the Bridges approach and examples of rehabilitation teams that have managed to make self-management support part of everything they do. She will talk about the possible mechanisms of impact when Bridges works, and how healthcare teams can sustain this way of working against every increasing demands.
