Overview

Learn to recognise, assess, and manage frailty to improve care, communication, and end-of-life support for older adults.

This interactive training session explores what frailty means, how to recognise and assess it using validated tools, and why early identification matters for patient wellbeing.

Participants will learn to manage frailty with a holistic, person-centred approach — addressing physical, psychological, and social needs.

The session also covers effective communication, advance care planning, and the use of frameworks such as the Rockwood Clinical Frailty Scale and ReSPECT to guide compassionate, appropriate care at the end of life.

This training is delivered in accordance with policies, procedures and guidelines used in Kent and Medway. Please check policies, procedures and guidelines within your own trust.
