Welcome to the Frimley / Surrey Heartlands ISDN Study Day! Join us for an exciting in-person event at Chertsey House, St. Peter’s Hospital. This event is open to colleagues working across the end-to-end stroke pathway in health and social care and the voluntary sector. The event is being combined with our Annual ISDN Forum and Quality Improvement / Audit competition. Intended learning outcomes and objectives include:
- To provide opportunities for shared practical learning, focusing on quality improvement.
- To celebrate the community of professionals working in stroke and provide opportunities for connecting with colleagues working across the stroke pathway and within the geography.
A full agenda will be shared shortly but speakers include Professor Martin James, Clinical Director, Stroke Programme, King’s College London; Matt Tite, Director at NHS Elect; Professor Deb Lowe, Medical Director at The Stroke Association and National Clinical Lead for Stroke Medicine – GIRFT Programme NHSE; and Michelle Long, National Speciality Advisor for Stroke at NHS England
Themes for the day will include:
• What we learn from listening - insights from the stroke survivor's lived experience of navigating the post stroke journey
• Making sense of the new SSNAP data set and scoring system
• Using SSNAP and other quality improvement tools to improve and develop your service
• Visual difficulties post stroke
• Data that delivers real-time improvement - using your acute stroke SSNAP results to drive improvement
• 'Matched' care and prioritisation approaches in rehabilitation and improving community integration
