Dyma ein cyfle unwaith mewn cenhedlaeth i drawsnewid iechyd, gofal a chymorth yng Ngorllewin Cymru.
Mae adnoddau sylweddol gan Gronfa Gofal Integredig a Chronfa Drawsnewid Llywodraeth Cymru, ymrwymiad clir gan y Bwrdd Partneriaeth Ranbarthol i sicrhau newid radical, strategaeth newydd tymor canolig Bwrdd Iechyd Prifysgol Hywel Dda yn seiliedig ar fodel cymdeithasol o iechyd, a hanes cryf o arloesi yn y rhanbarth oll yn golygu ein bod mewn sefyllfa dda i ddod â gwasanaethau ynghyd i fodloni anghenion unigolion a chymunedau, gan helpu pobl i barhau i fod yn iach a byw bywydau bodlon, hapus a llawn.
Bydd cynhadledd flynyddol Partneriaeth Gofal Gorllewin Cymru yn rhoi cyfle ichi ddarganfod mwy am ein siwrnai drawsnewid ac ystod o raglenni cyffrous sydd eisoes ar y gweill. Yn dilyn anerchiad agoriadol gan y Dirprwy Weinidog Iechyd a Gwasanaethau Cymdeithasol, bydd sesiynau rhyngweithiol gan arbenigwyr amrywiol yn rhoi cyfle ichi rannu eich syniadau a'ch safbwyntiau, ystyried sut y gallwch chi i lunio newid wrth iddo ddigwydd a rhwydweithio â chydweithwyr ar draws sectorau ac o wahanol rannau o'r rhanbarth.
Bydd y gynhadledd o fudd i uwch-reolwyr a rheolwyr canol, ymarferwyr, comisiynwyr a darparwyr. Hyn a hyn o lefydd sydd ar gael felly i osgoi cael eich siomi, archebwch le nawr.

Our once-in-a-generation opportunity to transform health, care and support in West Wales is here.
Significant resources from the Welsh Government’s Transformation and Integrated Care Funds, a clear commitment by the Regional Partnership Board to drive radical change, Hywel Dda University Health Board’s new medium term strategy based on a social model of health, and a strong record of innovation in the region all mean we are well placed to bring services together to meet the needs of individuals and communities, helping people to stay well and live fulfilled, happy and engaged lives.
The West Wales Care Partnership’s annual conference gives you the opportunity to find out more about our transformation journey and a range of exciting programmes already underway. Following an opening address by the Deputy Minister for Health and Social Services, interactive sessions by a variety of experts will give you the chance to share your ideas and perspectives, consider how you can shape change as it happens and network with colleagues across sectors and from different parts of the region.
The conference will benefit senior and middle managers, practitioners, commissioners and providers. Spaces are limited so to avoid disappointment book now.
