Greener Practice is delighted to have Clare Howard, a senior pharmacist with extensive experience leading national medicines safety work, particularly around polypharmacy, deliver this webinar. Clare was the lead author for the RPS Polypharmacy guidance and the lead author for the RCGP/RPS repeat prescribing toolkit, published in 2024.

For the last three and half years Clare has lead The Health Innovation Network Polypharmacy Programme. This innovative programme has gained national and international recognition for its combination of data, educational outreach and patient-facing work to turn the tide on problematic polypharmacy.

Join us for this session to hear from Clare about how The Health Innovation Network Polypharmacy Programme delivered real change by working in collaboration with Primary Care Prescribers, Geriatricians and patients. Clare will share how the programme was developed, delivered and evaluated to support General Practice and PCNs to improve the care we give to our older people and others exposed to harm from multiple medicines taking.

There will be time for a Q+A, as well as a brief introduction to Greener Practice.
