Health Initiatives 2017/18

Hartlepool and Stockton on Tees Clinical Commissioning Group (CCG) continue to recognise the value of the Voluntary, Community and Social Enterprise (VCSE) sector and the role it has to play in improving people’s health and wellbeing.
They are continuing to support the sector by working with Catalyst to commission innovative health and wellbeing projects across Stockton on Tees and Hartlepool.
The 2017/2018 Health Initiatives will deliver new and innovative projects from the 1st October 2017 that will have a direct impact on health and social care, aim to support people to be better informed about their health and wellbeing and empower them to make positive moves to change their lifestyles.
The application process will commence on Monday 27th March 2017 and will close on Friday 12th May 2017.
Shortlisted projects will be invited to deliver presentations during June 2017 and all successful projects will be awarded by the 1st July 2017.
The application forms will be available to download from the Catalyst website from Monday 27th March 2017.
