The Heart Failure Academy’s ‘Navigating the heart failure journey’ study days

The Heart Failure Academy, funded by NHS England, is a collaboration across all three Yorkshire and Humber Integrated Care Boards, Health Innovation Yorkshire & Humber and Leeds Teaching Hospitals NHS Trust, working with Pumping Marvellous.

Health care professionals from all sectors who are new to supporting people living with heart failure

Background

The Heart Failure Academy’s ‘Navigating the heart failure journey’ study days are an opportunity for health professionals new to heart failure to meet the local heart failure specialist teams and apply learning from the ‘Navigating the heart failure journey’ map to clinical scenarios, guided by the local heart failure specialist teams.

These sessions are multidisciplinary, providing a fantastic opportunity to network and share best practice. We will examine what the challenges are to the local system and how we can work together across sectors to support people living with heart failure to get the best outcomes.

The content of the study days will be the same, tailored to each region (Humber North Yorkshire, South Yorkshire and West Yorkshire). Each session will be supported by local heart failure experts who support people living with heart failure across the whole heart failure pathway.

Aim

The aim of this study day is to support healthcare professionals to develop their knowledge and skills to provide a person centred approach to recognising the signs and symptoms of heart failure, enabling a timelier diagnosis, swift escalation to the right treatment at the right time.

Learning outcomes

At the end of the session, healthcare professionals will

• develop their confidence in recognising the signs and symptoms of heart failure

• be able to describe the appropriate referral of people with suspected heart failure

• be able to summarise the management of different heart failure phenotypes

• apply a person centred evidence-based heart failure therapy to clinical scenarios

• develop advance care planning consultation skills
