Description

The national Building Health Partnerships programme is working with the Calderdale and Wakefield Health and Care Systems and the Voluntary and Community Sector to improve the health of people across Calderdale and Wakefield.

The aim of the programme is to build cross sector collaboration, community engagement and co-creation around two specific areas:

  • Musculoskeletal conditions in Calderdale (Self-Care Management and Recovery & Rehabilitation)

  • Eye Health in Wakefield (Prevention)

With the ambition of creating a shared sense of responsibility between health and care organisations and the local population to improve population health outcomes in these two areas.

This is the first of four partnership events that will focus on what’s needed to improve the lives of people with Musculoskeletal (joints and bones) conditions and how a collaborative, partnership approach can help. Statistics suggest that 26% of people (40,000) living in Calderdale do less than half an hour of exercise a week and we know exercise can reduce hip fractures by up to 68%.

  • Find out more about the Building Health Partnerships programme

  • Help us develop a collective understanding of MSK conditions and the services available

  • Start to develop a vision and work plan for how things could be better

Help us work out who our partners ought to be, focusing on co-production, partnership working and team working and identifying what we don’t know

This is a great opportunity get involved in a project which will have a real impact on the experience of patients across Calderdale, ensuring we work together to get people active, improving musculoskeletal (joints and bones) health and outcomes for the people of Calderdale.

We’ll be holding a second session to launch the Building Health Partnerships work on eye care in Wakefield in early December to help develop an awareness raising programme with the Voluntary and Community Sector (community groups) and interested individuals, to give Wakefield residents the knowledge and skills to look after and retain their sight.
