With our project Partners - SGN, Scottish & Southern Electricity Networks, Southern Water, South East Water and the NHS Hampshire & Isle of Wight Integrated Care Board - Citizens Advice Hampshire are pleased to invite you to our very special event being held to celebrate the fifth anniversary of the successful Home & Well project.
Established in 2020, the aim of the Home & Well project continues to be to support vulnerable people, helping to ensure that they that feel safe and comfortable in their home environment. To achieve this, specially trained Citizens Advice advisers combine all the elements of helping clients with household fuel or water poverty and identifying cases where being registered for the Priority Services Register is essential. We also provide a holistic wrap around approach to care and actively signpost Home & Well clients to services that tackle additional issues such as concerns with housing, employment, debt, loneliness and managing relationships.
With support from our Partners, our award-winning project continues to expand and now takes referrals on a direct basis, in addition to Primary and Secondary Care NHS referrals. We increasingly work with local Social Prescribers and many other Hampshire based charities.
With plenty of time for networking, this celebratory event is a chance to both connect with like-minded individuals and organisations and to meet the Partners. A range of organisations will also be sharing information via the ever-popular market stalls. Come along and hear from University Hospital Southampton, The British Red Cross and Home & Well Advisers about how this amazing project works and the difference it makes to people’s lives countywide.
