Future Model of Care in Hounslow
Work has been continuing to develop a new model of care in Hounslow that aims to provide integrated, proactive, and holistic care through new multi-disciplinary teams.
Significant progress has been made in shaping the model, which will begin a 12-month Test and Learn phase from September 2025, supporting five GP practices in the Hounslow Health PCN. The programme will address challenges in the current system by shifting to a model where residents engage with one team rather than many services.
Following the Health and Social Care Forum meeting in June this meeting will update on the progress of the project.
