Investigations are a vital part of good governance in social care provision. Robust investigations identify the root cause of an event and lead to improved service provision. However, many investigations are inherently flawed because they are conducted by people who do not have the appropriate knowledge and skills. This can lead to unsafe practice, unfair dismissal and regulatory non-compliance.
