How to transform the Continuing Healthcare & Decision Support Tool process

Stop the paper chase, reduce duplication, reduce Delayed Transfers of Care and improve hospital discharges!
The Continuing Healthcare assessment process today is primarily a paper based process with many forms, emails and faxes being used; the Continuing Healthcare Checklist and the Decision Support Toolkit being primary examples.
As a result, professionals at the centre of the Continuing Healthcare process struggle to synthesise the information from the reams of paper they receive. Progress is laborious and actions repetitive.
Paper handling causes unacceptable levels of delay and ensures that service provision to the citizen and carers is sub-optimal. One NHS FT identified 17 days of delay built-in to their CHC & DST process as a direct result of the inefficiency of paper handling.
IEG4 has been awarded by NHS England an SBRI funded contract to develop a solution for hospital discharge focusing initially in Continuing Healthcare (CHC).
Come along to this event to hear progress on the CHC solution and the overall architecture for a software solution to improve the efficiency of hospital discharge from the NHS frontline staff and experience a demonstration of the CHC assessment solution.
