Improving the Quality of Life for People with Frailty: Northern Meeting, covering Yorkshire, Humber and North West Regions.

This is one in a series of regionally led events supported by NHS England to enable local commissioners, clinicians, providers and others to improve the care for older people living with frailty by coming together to consider local challenges and how these can be addressed.

Led by Yorkshire & Humber AHSN the event will support a problem-solving approach to improving the health and well-being of older people living with frailty in line with the challenges of an ageing population. The event will address frailty by building connectivity to local teams and start developing networks to support STP frailty leads in improving the quality of life for people with frailty.

This practical day brings together key national and regional stakeholders who want to improve services for people with frailty - including STP Leads, commissioners, clinicians from all relevant disciplines, primary, secondary and community care, across STP footprints to understand what’s working well, what’s not and to explore data sources and national tools to help us gain insight to make much needed improvements.

The aims of the event are:

Focus national strategic plans for older people living with or at risk of developing frailty onto the STPs in your regional footprints.
Align strategic planning and prioritisation focused on healthy ageing through to acute frailty management, long term and end of life care for older people across the region
Share good practice more assertively and capitalise on what is working well in their region, where and why
Identify regional variation and where key elements of frailty care are either missing or could be assisted to develop
Explore workforce issues for example the role of extensivism in primary care
Understand how we can best connect the national programme and STPs around frailty and older people going forwards.
