This event is part of the NHS Resolution series of free interactive safety and learning events for our members, and will provide an opportunity to explore some of the issues and impact of harms leading to inquests.

Places are limited and interest in our previous sharing and learning events has been very high. Attendance is determined on a ‘first-come’ ‘first-serve’ basis – so don’t delay!

Inquests – what lies beneath event will be an exploration of the issues and impact of harms leading to inquests including:

  • What emotions/anxiety does an inquest evoke?

  • Helping participants learn from each other

  • Identifying what families want from the process

  • Maximising learning opportunities

Afternoon breakout sessions will provide opportunities for smaller group discussions with experts to explore post incident management, what to expect when attending an inquest and the importance of openness and candour.

  • Dr Reshad Khodabocus - Consultant Emergency Medicine The Mid Yorkshire Hospitals NHS Trust

  • Alan Gallagher - Head of Risk North East Ambulance Service NHS Foundation Trust

Breakout session overview:

Breakout Session 1 - Alan Gallagher workshop

The workshop will be based upon an actual case which led to the development of Family Liaison Officers within North East Ambulance Service, moving onto provide details of our experience and the wider benefits provided by the role.

Breakout Session 2 - Area Coroner for Cumbria workshop

The talk will cover:

  • Coroners jurisdiction and the duty to investigate deaths

  • Death referrals & natural vs unnatural deaths

  • The duty to report deaths and completing the Medical Certificate of Cause of Death (MCCD)

  • Cases that should be referred to the Coroner

  • The inquest process and particularly;
    - Hospital deaths
    - How to prepare for an inquest / writing witness statements and what to expect at court
    - Root Cause Analysis Reports and Regulation 28 Reports to Prevent Future Deaths

Breakout Session 3 - Superintendent Marraty workshop

A discussion of Post Incident Procedures, used within the Police Service and their application to NHS practices.

This free training event will benefit staff ranging from chief executives and directors to frontline clinicians and designated patient safety leads and will be of particular benefit to NHS staff who attend inquests or who are responsible for conducting or overseeing investigations of patient deaths, as well as those whose role involves helping families through the process
