Sheffield Clinical Commissioning Group (CCG) has been successfully chosen to be an early adopter
site, for IPC. Sheffield CCG currently delivers Personal Health Budgets (PHB) to people with continuing health care and is now looking to role this out further, being an early adaptor site means we can bring this vision forward to 2018. IPC is a new approach in joining up health, social care and other services at the level of each individual.
