This course is designed for Health and Social Care staff working in Somerset to introduce the importance of advance care planning and the Somerset treatment escalation plan.


It will help you:


  • To identify and discuss the different types of advance care plans that can be made, including plans which are a legal document, such as an Advance Decision to Refuse Treatment and a Power of Attorney.
  • How and where these plans can be created.
  • To explore best practice in having planning ahead conversations with others
  • To increase understanding of the responsibilities in discussing, sharing, and updating an advance care plan.
  • What plans are saved and can be accessed on Somerset’s shared integrated digital e-Record (SIDeR)
  • Please note there is no visitor parking in Wellington Hospital - the nearest public parking is South Street care park opposite the hospital.
