Guardians are required to complete an Activities Consent Form, otherwise the booking will be void and the activity will be unable to go ahead.

Upon completion of the form, and before the activitiy takes place, please forward to Workington.LC@GLL.ORG. If more than one activity is booked then only one form needs to be completed.
