About this event

We welcome you to the next Learning from Deaths event taking place on Wednesday 8th October 2025 at the Health and Care Academy, NSECH.

The event plans to bring together multidisciplinary teams of all levels to look at ‘Learning from Deaths - The Surgical Experience’

  • All parts of the patients journey to surgery including the Emergency Department, Ambulatory care, Theatre and Surgical wards.
  • Case presentations and review of the different types of processes.

Like previous events, a thematic analysis of the data we have collected so far will be used to inform presentations. Tabletop interactive sessions will then follow to ensure learning and improvements are taken forward.

The event is open for up to 80 people to attend.

We are encouraging multidisciplinary team attendance plus members of Trust Board, Patient Safety Partners and the Senior Leadership Team are also invited to share in the event which is an important part of the Safety, Quality and Improvement Committee priority workstreams and the wider Patient Safety Strategy.

We would like people from all parts of the journey to attend and help us improve the pathway.
