Learning Lessons - Mr V and Mr W
THIS IS A FREE MULTI-AGENCY TRAINING SESSION
Mr V and Mr W were both residents of the Isle of Wight who were using health and social care services at the time of their deaths. Mr V was 72 years old when he died at his own home in Newport in December 2014. Mr W was 86 years old when he died at St Mary's Hospital in August 2013.
Independently commissioned reviews into the circumstances leading to both deaths can be found on the website of the Isle of Wight Safeguarding Adults Board www.iwight.com/SAB. Both gentlemen appeared to have been ‘difficult to engage’ with and self neglect was certainly a feature of both of their lives, and their deaths.
The Isle of Wight SAB has commissioned 3 separate half day sessions to enable professionals to learn lessons from these two cases. The purpose is not to apportion blame, but rather to see how we can learn from these sad and difficult events to improve practice for users of services in the future.
Each session will be facilitated by David Thornicroft, of St Thomas Training, who was the author of the review into the death of Mr V, and who has substantial experience of dealing with such issues. St Thomas Training is currently Isle of Wight Council's approved provider of training in Mental Capacity Act and Safeguarding Adults.
PLEASE NOTE THAT THIS VENUE IS NOT LOCATED NEAR SHOPS OR FOOD OUTLETS, SO WE WOULD ADVISE YOU TO BRING LUNCH WITH YOU. TEA AND COFFEE WILL BE PROVIDED. THERE WILL BE A CHARGE OF £50 FOR NON-ATTENDANCE.
