Living with Dementia & Advanced Care Planning

This is a free seminar for patients and carers within the four surgeries of the Welwyn Garden City Primary Care Network.There will be presentations from an NHS Consultant looking at the various forms of Dementia, followed by a presentation by Dr Tom Gilham talking about the need for advanced care planning. Parking is available on site, as well as adjacent NHS offices and on the road. Free tea or coffee will be provided.
