  • Learn who should be involved in transitions planning, how, and when
  • Understand more about your role in the process, and that of your young person
  • Consider some of the possible options
  • Look at examples of transition plans and take home a template to help your own planning
  • Discover more sources of information and support

Led by:

Tracey Francis (Transitions Officer, Contact a Family) and Rebecca Scarlett (Senior Policy and Information Officer, Lead Scotland), working with Signpost and the West Lothian Parent/Carer Alliance.
