Description
Within each of the new Health and Social Care Localities, the Edinburgh Health and Social Care Partnership (also known as the Integrated Joint Board or IJB) is developing ‘Locality Hubs’ which will bring together community staff from all sectors and a wide range of disciplines.
The key focus for the new locality hubs will be delayed discharges for older people and finding new ways to ensure that older people remain in hospital for the shortest period possible and to strengthen the support within the community on their return.
As part of this development, EVOC, The Eric Liddell Centre, Health in Mind and Libertus will be delivering an exciting new Hospital Discharge Support Project which will see a team of staff placed within the locality hubs and in acute hospital settings.
We believe that this work will be a first in Scotland in that it builds a single, coherent interface for the Third Sector into the hospital environment, with the support of both Health and Social Care colleague, and which will see the delivery of an end-to-end service to improve discharge efficiency and community supports to older people.
We will be hosting a number of engagement events over the course of the next few months to introduce the project and to ensure that we are able to engage the widest range of partners.
The first of these events will be on the 16th of May and will be for Statutory colleagues and National Third Sector providers.
We will be holding locality based events towards the end of May and beginning of June that will be posted on here shortly.
