Good practice in the community is being celebrated in an annual awards scheme, with potential winners being reminded they have just a few weeks left to make their nomination.

Medway Clinical Commissioning Group (CCG) invites members of the NHS, as well as voluntary and community groups, to enter for awards being presented at the Patient and Community Partnership Awards - 'Keeping Medway Active and Well' in September 2019.

The awards are split into two categories: Maintaining wellbeing in the community– How services/providers are supporting wellbeing in the community by promoting fitness; and supporting people to look after themselves, where services/projects show what they are delivering to support health and wellbeing for people in Medway.

Nominations close on 3rd May 2019, and shortlisted entrants will be invited to give a short presentation at the event, with an overall winner decided on the day.

The afternoon, 4pm-6pm will be in the style of a health fair/market place to showcase projects/initiatives which involve getting the people of Medway active. The evening, 6pm-8pm, will be used to present awards to the winning nominees under the above categories.

Last year the Supporting Community Health award went to a small grants awarded community group and we would really welcome nominations from similar organisations at this year’s awards.

Submit nominations for the award categories with a brief outline of the project including innovation and/or community involvement, impact/outcomes and sustainability for patients no later than Friday 3rd May 2019. Shortlisted nominees will be announced by Friday 17th May 2019 and overall winners will be announced at the event.
