Description

The Annual General Meeting of Medway Voluntary Action will take place on 22nd November 2017 at the St George Hotel, 8 New Road Avenue, Chatham ME4 6BB.

The MVA AGM will run from 09.00-10.00. This will immediately be followed by a session on Social Prescribing and a new Health initiative, with Guest Speakers from Medway Council's Public Health team and the NHS Improvement Programme.

During the AGM we will also be presenting to the winner of this year's Barry Clout Outstanding Contribution to the Medway Voluntary Sector Award 2017. MVA will also be receiving the REVAMP Award (Recognition of Excellent Volunteer & Management Practice).

1. Introduction

2. Health and Social care profiles for the Medway Model

Dr David Whiting, Consultant in Public Health, Public Health Directorate, Medway Council

Medway is developing an exciting new approach to improve the quality of care that is based around a concept called the Medway Model. The Medway Public Health team has developed a set of locality profiles that show the key needs in each of the six “sub-hubs” of the Medway Model, and this presentation will provide a brief overview of each area.

3. Opportunities for Medway

A wide range of Medway partners are seeking to develop a Medway Social Prescribing project, building on the positive existing work that is already in place. Opportunities are available to bid for external funds to support this. Medway Public Health welcome the opportunity to engage with a wide range of voluntary and third sector partners at the earliest stage of planning the project. The Medway Public Health team will provide a short overview of what social prescribing is and would welcome your input into how a successful project could be developed to help Medway residents lead healthier happier lives.
Breakout sessions will be available to investigate partnership opportunities and funding

4. Joining the dots – how the VCS can work with Health and Social Care to achieve national Health priorities

How can we ensure that we keep people at home and well supported, reducing the demand on resources that are becoming increasingly scarce? This session will help us to understand the national focus on patient flow and the importance of patients’ time as the most important currency in health and social care and the impact the VCS can have on this agenda.
