For many years, it has been established practice across health and social care for people who need to be hoisted, or cared for in bed, to have a care package with two people to assist and carry out the care. It is unknown where or how, this practice became established, but with innovations in moving and handling equipment and a move to more person-centred care & support, this requirement is increasingly being questioned, as is the application of blanket policies across agencies that do not take into account personalisation or the skills and knowledge of staff.

As an Occupational Therapist I have been appointed to work on our Moving with Dignity project (right-sized care & support) within the Review and Reassessment Team. I have been in post since January 2019 and we are now in a position to move forward with practical plans to implement changes to the way Denbighshire residents are assessed for and receive moving and handling support

Aims of the Project

•To increase the independence of the citizen, making them an active participant in their care, thereby improving their well-being, privacy and dignity.

•To invest in new advances in moving and handling equipment

•To develop and consolidate a collaborative approach to the ‘Moving with Dignity’ project

•To ensure staff are competent and confident in identifying those individuals whose needs could be best met through single handled care.

Denbighshire aims to achieve this through:-

•The delivery of an Engagement day, to include key stake holders and specialist equipment providers.

•Two half day sessions will be held on the 15th October, 2019 at Oriel House (9am - 12.30 and 13.00 - 16.30)The delivery of a specialist training course for risk assessors (Occupational Therapists and Physiotherapists from Health and Social Services), District Nurses and key personnel in each domiciliary care agency.

•The provision of ongoing support and access to a regularly updated online resource.

Fel Therapydd Galwedigaethol, rwyf wedi cael fy mhenodi i weithio ar ein prosiect Symud ag Urddas (gofal a chefnogaeth maint cywir) o fewn Tîm Adolygu ac Ailasesu. Rwyf wedi bod yn y swydd ers mis Ionawr 2019, a rydym mewn sefyllfa bellach i fwrw ymlaen â chynlluniau ymarferol i weithredu newidiadau i’r ffyrdd y mae preswylwyr Sir Ddinbych yn cael eu hasesu ac i gael cefnogaeth symud a chodi.

Nodau'r Prosiect

• Cynyddu annibyniaeth y dinesydd, gan eu gwneud yn gyfranogwyr gweithredol yn eu gofal ac felly yn gwella eu lles, preifatrwydd ac urddas.

• Buddsoddi mewn cyfarpar symud a chodi newydd.

• Datblygu a chyfuno dull cydweithredol i’r prosiect ‘Symud ag Urddas’.

• Sicrhau bod staff yn gymwys a hyderus i ganfod yr unigolion hynny lle gallai eu hanghenion gael eu bodloni’n well gan ofal unigol.

Nod Sir Ddinbych yw cyflawni hyn drwy:-

• Darparu Diwrnod Ymgysylltu, i gynnwys rhanddeiliaid allweddol a darparwyr cyfarpar arbenigol.

• Bydd dwy sesiwn hanner diwrnod yn cael eu cynnal ar 15 Hydref, 2019 yn Nhŷ Oriel (9am - 12.30 and 13.00 - 16.30). Darparu cwrs hyfforddiant arbenigol ar gyfer aseswyr risg (Therapyddion Galwedigaethol a Ffisiotherapyddion o Wasanaethau Iechyd a Chymdeithasol), Nyrsys Ardal a staff allweddol ym mhob asiantaeth gofal cartref.

• Darparu cymorth a mynediad parhaus i adnoddau ar-lein wedi’u diweddaru’n rheolaidd.
