Description

The London Urgent and Emergency Care Improvement Collaborative has been engaging with multiple stakeholders from acute and community trusts around improving patient flow and discharge since our launch on 04 July 2017. A recurrent theme being fed back from A&E systems is the challenge acute care providers face accessing neuro-rehabilitation beds for patients. We understand from the data provided by acute trusts that this cohort of patients experience many delays in their care pathways both in and out of hospital.

We understand acute care and rehabilitation care providers are concerned with discharge planning processes in both acute and community hospital settings when patients are ready for their next step of the neuro-rehabilitation pathway journey. These delays appear to be having a negative impact on patients requiring specialised commissioned services.

We would like to explore opportunities in working collaboratively across London to improve neuro-rehabilitation patient pathways, flow and discharge. We welcome the opportunity to explore the challenges described by both acute and specialised rehabilitation providers in managing neuro-rehabilitation patient discharge processes and pathways in hospital and into specialised commissioning beds and subsequently into the community.

Currently the London Urgent and Emergency Care Improvement Collaborative team offers support to acute, mental health and community providers across London. We would like to explore possible solutions to the challenges identified with neuro rehabilitation pathways with specialised commissioners in NHSE, with neuro- rehabilitation providers, CCGs and Local Authorities from London and the Home Counties.
